Job Description:

Duties include, but are not limited to, the following: – May be required to drive long distances for placement, appointments, visits for the individuals – Escorts individuals to appointments, such as doctors’ visits, leisure/ recreational activities, and discharges, as necessary.

Primary Duties & Responsibilities:

Utilizes all appropriate security and safety precautions according to established guidelines.
Provides basic patient care and works with individuals in a mental health setting to develop daily living skills.
Completes required documentation in patient and unit records.
Takes vital signs as needed and documents accordingly in the Electronic Medical Record – Ensures safety and security of individuals.
Observes and documents patient data, behavior, and activity.
