Intake Coordinator- Licensure Required- Thomasville

Performs social services function at an advanced level to people applying for Intellectual/Developmental Disability Services, their families and the agencies that support them.
Primary Duties and Responsibilities Include:
• Ensure completion of all aspects of the intake process including, but not limited to, review of applications and associated documents
• Follow up to obtain missing or incomplete information, eligibility letters, referrals for those who are deemed ineligible, tracking of completed screenings, update and assign to Planning List administration in IDD-C, update and maintain IDD-C tracking for all applications
• Communication with and education of families and individuals seeking services on the eligibility process and the evidence necessary for determining eligibility
• Act as liaison with local school systems and other stakeholders to share and explain available resources and how to access those resources
• May develop, arrange, and present training requested by community partners to create broader understanding of IDD services, waivers, and the process by which one obtains services
Minimum Qualifications
Master’s degree in a related area from an accredited college or university AND licensure in the area of assignment AND Two years of related post-license experience OR Master’s degree in a related area from an accredited college or university AND licensure in the area of assignment AND Two years of experience at the lower level Social Worker, Licensed 2 (HCP051) or position equivalent.
Preferred Qualifications Include:
• Licensed Clinical Social Worker, Licensed Professional Counselor or Licensed Marriage and Family Therapist and two years of post-licensure experience providing counseling, case management, assessments for court depositions, legal proceedings and/or eligibility determinations.
• Experience working with people with intellectual disabilities, knowledge of Medicaid Waiver and State Funded Services, as well as extensive use of electronic communication, use of state-wide databased systems and knowledge of social service agencies and local community resources is beneficial.
• Two years of experience providing case management and coordination services to individuals with I/DD in human services settings requiring use of clinical assessments, interviewing, and/or written reports of assessment findings
