Job Coach – New Hope

Job Summary:
Under general supervision, plans, manages, and evaluates social services programs designed to assist individuals in residential care to obtain the necessary occupational skills to transition to community living. Establishes and maintains relationships with other agencies and organizations in the community to meet needs and prevent duplication of services.

Primary Duties & Responsibilities Include:
Administers and coordinates the services and program activities in dependency, mental illness, youth development, or physical or developmental disability programs.
Guides and oversees the distribution of resources.
Identifies and/or manages grants, contracts, and other facets associated with the programs.
Implements, coordinates, monitor and evaluate social services programs.
Organizes community stakeholders and resources to identify proactive and effective solutions.
Evaluates the provision and quality of recommended services to assure compliance with program requirements and policies.
